Important Notes Before Completing the Form:
If you have more than one child enrolled in an HCDSB school, please complete a separate form for each child.
Only one parent/guardian is required to complete a form for each child.
You will need your child’s Ontario Education Number (OEN) to submit this form. The OEN can be found on a recent report card, the top of your verification form, or by contacting the school office.
Please ensure you are submitting your child’s preferred name or the name you provided at registration.
The form can be completed using a computer, smartphone, or tablet.
Sacramental Preparation Update
We are working with the Diocese of Hamilton and the Vicar of Education to support the sacramental preparation of our Gr. 2 and Gr. 7 students. As soon as the plan is finalized, we will inform parents and provide details.
Our Virtual Curriculum Night is scheduled for Wednesday, September 29th. The Administration Team will begin the evening with a welcome message at 6:30 pm followed by two classroom presentation times – 6:40 to 7:00 pm & 7:10 to 7:30 pm. Teaching staff will be using the Microsoft Teams for their virtual meeting – teachers will be posting the TEAMS link to your child’s D2L shell or emailing it directly to parents.
IEP Consulation Form
On Monday, September 20th, parents of students with IEPs will be emailed a Virtual IEP Consultation Form via the email we have on file. Please complete the form promptly. If you have a student who is on an IEP and don’t receive an email regarding the Virtual IEP Consultation Form, please notify Mr. Simon (email@example.com). Thank you!
In honour of our commitment to Truth and Reconciliation through Indigenous Education, we will be hosting Orange Shirt Day on September 30th. We recognize the harm residential schools caused to our Indigenous people and we honour those that survived and remember those that did not. We recognize the ongoing effects of intergenerational trauma caused by residential schooling and we wear an orange shirt in support of those who have been impacted by the harm of residential schools. Kids Red Cross/Safety First Courses
‘Kids Safety First’ – a Canadian Red Cross Training Partner – is offering a number of First Aid for Kids programs (including the Canadian Red Cross Babysitting Course) geared towards different grade levels. All courses are virtual. Please view the flyer here for more information.
