SPECIAL LUNCH PROGRAMS (2019-2020):  The Special Lunch Programs will begin the week of Monday, Sept 30th and will run until the week of June 15th inclusive. New this year is that for our Milk, Pizza and Salad Bar, you will be ordering for the entire school year using our School Cash Online System – you will have the option to pay the full amount or pay in up to 3 equal installments starting in September and coming out monthly. The deadline to order is midnight on Friday, Sept 20/19. DO NOT miss the deadline as our parent volunteers need to tally all orders and submit to the vendors. With the Lunch Box and Lunch Lady programs, you will order directly from the company using their links as provided on the Information Letter below.

TRANSPORTATION FORMS:  If your child is assigned to a bus but you do not want them to use it, please complete this form and submit to the office ASAP. We will advise Transportation of your decision and know that this can be reversed should your situation change. Some of our buses are at capacity, so this information allows us to have a more accurate count of users.

INTERESTED IN VOLUNTEERING AT SHOJ? – We must have a Criminal Reference Check on file in order for you to volunteer at the school in any capacity. Once a copy of the form has been placed on file, you will receive a “Declaration Form” each June, to be completed in preparation for the start-up of a new year. If you have not sent yours in, please print one off and return to the school office.

STUDENT ACCIDENT INSURANCE

The Halton Catholic District School Board does not provide accident insurance coverage for student injuries that occur on school premises or during school activities. Since accidents can happen, and some injuries result in medical, dental or other expenses that are not covered by provincial health care or employer group plans, as a parent, you may wish to consider purchasing student accident insurance.
Student accident insurance can provide you with financial piece of mind against the possible expenses that can be incurred if a child is injured at school.
The Board makes available an Accident and Life Insurance Program for students exclusively through Old Republic/Reliable Life Insurance Company. Participation is voluntary and the costs for the program are paid by the parent or guardian. The program offers a variety of plans and benefits, which can be tailored to suit your needs. The coverage provided by the policy is comprehensive and reasonably priced ($13.00 to $33.00 per year depending on plan chosen).

MEDIC ALERT: NO CHILD WITHOUT PROGRAM

If your child has a medical condition, he or she may be eligible for a completely subsidized MedicAlert ID and service plan through the No Child Without program. It’s offered free of charge to children ages 4 to their 14th birthday. Should an emergency occur at school, or anywhere, emergency responders and health professionals will have immediate access to your child’s medical records and you’ll be notified.

The No Child Without program is offered through participating schools. Over 6,000 public and Catholic schools across Canada are already enrolled. Sacred Heart of Jesus is a registered school.

Visit the enrollment page to obtain a set of bar code and PIN number or obtain a No Child Without brochure from the school. Each set of bar code and PIN numbers is unique and they are assigned to the registered school only. A separate set of bar code and PIN number is required for each child you plan to enroll in the program.
